Error Fieldname must not be empty

In Zapier, the format for filling PDF form field name is pageNumber;fieldName;value. These parameters are separated by a semicolon.

When you get the Fieldname must not be empty error message, please check if you are using a semicolon and the field name is added in the correct order.
